Thank you for your interest in Simply EZ HDM of Northwest Ohio, LLC. Our office hours are Monday-Friday from 8:00am to 4:00pm Eastern Time.

If you are not affiliated with Passport, Title III C, or Ohio Home Care programs, you may purchase meals direct from us. Five meals a week (our minimum) is only $30.00. Seven meals are $40.00, or you may order any combination of those amounts. If you would like to get started please contact our office toll free at 1-866-618-3963.
